Paracetamol
Dosage/Direction for Use
120 mg/5 mL Childn 4-6 mth 2 droppers (2.4 mL), 2-3 mth 1½ droppers (1.8 mL), 1 mth 1 dropper (1.2 mL). 250 mg/5 mL Childn weighing >34-50 kg 2 tsp (10 mL), >25-34 kg 1½ tsp (7.5 mL), >17-25 kg 1 tsp (5 mL), >13-17 kg ¾ tsp (3.75 mL), 9-13 kg ½ tsp (2.5 mL). All doses to be given every 4 hr, not >5 times daily.
Administration
May be taken with or without food.
Contraindications
Hypersensitivity. Not to be used for >5 days continuously. Severe hepatic impairment or severe active liver disease.
Special Precautions
Discontinue immediately if symptomatic allergy or hypersensitivity reaction (eg, swelling of face, eyes & lips, urticaria, fainting, skin rash & peeling) occurs. Do not take >5 days; other paracetamol containing products. Limit use to <4 g daily. Increased risk of liver damage in patients consuming ≥3 alcoholic drinks daily; alcoholism. Severe hepatotoxicity on acute overdose & liver damage in adults w/ chronic daily dosing. Patients w/ alcoholic & active liver disease; G6PD deficiency. Hepatic & severe renal impairment. Pregnancy & lactation.
Adverse Reactions
Skin rash. 120 mg/5 mL: Increased Cl, uric acid & glucose, decreased Na, bicarbonate & Ca; anemia, blood dyscrasias (eg, neutropenia, pancytopenia, leukopenia); increased bilirubin & alkaline phosphatase; increased ammonia, nephrotoxicity w/ chronic overdose, analgesic nephropathy. 250 mg/5 mL: Cutaneous hypersensitivity reactions including angioedema, SJS/TEN.
Drug Interactions
Increased levels/effects of aripiprazole, busulfan, dasatinib, imatinib, lomitapide, pimozide, prilocaine, sorafenib, vit K antagonists. Decreased level/effects w/ anticonvulsants (hydantoin), barbiturates, carbamazepine, cholestyramine resin, peginterferon α-2b. Increased risk of paracetamol-induced hepatotoxicity w/ ethanol. 120 mg/5 mL: Increased levels/effects w/ dasatinib, imatinib, INH, metyrapone, probenecid, sorafenib. Decreased rate of absorption w/ food. Decreased levels w/ St. John's wort.
